The site will ask you a series of questions about your adult ward's situation and then create an Annual Report based on YOUR ANSWERS. This report can then be printed and filed in your Probate Court.    Financial Power of Attorney
The Georgia General Assembly voted in 1995 to establish within the Official Code of Georgia Annotated, at Section 10-6-140, a statutory form for the Financial Power of Attorney. The form that is set out in the statute may be used to create a Financial Power of Attorney. This form is merely a suggestion and is not the only form permitted to create a Financial Power of Attorney.

   Pauper's Affidavit (Request to File without Paying Fees
The Pauper's Affidavit, also known as a "pauperis" affidavit, can be filed by very low-income persons to avoid paying filing fees to the court. Usually the judge will review the affidavit and make a decision about whether you have to pay fees or not. If you file this affidavit, you must be ready to respond to the judge about your income.

   The Georgia Statutory Financial Power of Attorney
During the 1995 Georgia General Assembly, the Legislature voted to establish within the Official Code of Georgia Annotated, at ' 10-6-140, a statutory form for the Financial Power of Attorney. The form that is set out in the statute may be used to create a Financial Power of Attorney. This form is merely a suggestion and is not the only form permitted to create a Financial Power of Attorney. The information attached includes an explanation of the Financial Power of Attorney, cautionary language to those considering giving someone power over their financial affairs and instructions for completing the statutory form. Finally, attached is a copy of the form substantially the same as the form which appears in the Official Code of Georgia Annotated.
