   Immigration Forms, Fees and Fingerprints (Separate Website)
Welcome to the Forms, Fees and Fingerprints information center. From here you'll be able to find most of what you'll need to apply to for a immigration-related service, benefit, or permit. Many of these forms (benefit applications and petitions) require the payment of a fee in order for BCIS to process (adjudicate) your requests. Some also require applicants to pay an additional and separate fee for fingerprinting. This section will provide you with information on fees and fingerprint requirements. The Forms and Fees page provides information on Immigration forms and how to print them.

   Refugee Resettlement Program Application (Separate Website)
The Refugee Resettlement Program is a federally-funded program that provides cash assistance, medical assistance, health screening, and social services to refugees. Refugee cash and medical assistance is administered through county DFCS offices. The health screening is provided through county health departments. Social services are provided by a variety of contract agencies. These web pages provide a Program Overview and Information about How To Apply for services.

This web site contains a large number of forms available in PDF format, including, but not limited to: (1) Benefit Claims Forms, (2) Benefit Claims Supporting Forms, (3) Direct Deposit Forms, (4) Certain Internal Revenue Service Forms. The web site also allows you to apply on line for certain benefits and services.

   TANF/Food Stamps/Medicaid Application (Separate Website)
This is an interactive program to assist people in completing the initial application for TANF, Food Stamps and Medicaid. You must user INTERNET EXPLORER to access this program. The initial application is the first step in the application process. This program will take you to another website where you will register, login and then complete an interview.
